David
Moore, M.D., earned his undergraduate degree in
biology and chemistry from the University of Delaware.
He received his medical degree from Jefferson Medical
College in Philadelphia and completed his residency at
Wyoming Valley Family Practice. Board Certified in
family medicine, Dr. Moore was a member of the Beta Beta
Beta Honor Society at the University of Delaware and the
Omar Harre Honor Society at Jefferson Medical College.
Prior to joining the Neighborhood Healthcare Center in
Cumming in October 1994, Dr. Moore worked at the
Neighborhood Healthcare Center in White County, a center
he helped open in 1987. Before that, he practiced in
Helen at the Village Health Clinic. Dr. Moore lives in
Forsyth County, is happily married and has 3 children
and a dog. His wife is a school nurse. For hobbies he
enjoys building computers, restoring a '68 Mustang and
jogging competitively in 10K races. He is very active as
a Boy Scout Leader, camping and backpacking.
Alisa
Allaire, MD, PhD, has joined The Neighborhood
Healthcare Center in Forsyth as a primary care
physician. Dr. Allaire earned her medical degree at the
Emory University School of Medicine in Atlanta where she
also completed her residency. In addition, Dr. Allaire
has a PhD in Exercise Physiology from Temple University
in Philadelphia, Penn. She is certified by the American
Board of Internal Medicine. Before joining The
Neighborhood Healthcare Center, Dr. Allaire practiced
with the National Naval Medical Center in Bethesda, Md.;
Walter Reed Army Medical Center in Takoma Park, Md.; and
Triangle Community Physicians in Durham, NC.
Jan
Smith, M.D., earned her bachelor's degrees in
biochemistry and biology from the University of Georgia.
She received her master's degree from Georgia State
University in microbiology and attended Emory University
where she received her medical degree. Dr. Smith
completed her internship and residency at Halifax
Medical Center in Daytona Beach, Florida in family
practice. She is board certified in family practice. She
is a member of the Georgia Academy of Family Practice,
the American Academy of Family Practice, and the Medical
Association of Georgia. She lives in North Forsyth
County. She attends First Baptist Atlanta and is active
with the Georgia Department of Agriculture's Equine
Impound Program. |
